Understanding health insurance in China is essential for any foreigner staying long-term. This guide covers the public system, commercial options, and how to make claims.

Employee Basic Medical Insurance

Under Chinese law, employed foreigners must participate in social insurance including basic medical insurance:

• Employer must register you within 30 days of employment

• Applies if you hold a Work Permit, residence permit, or Permanent Resident ID

• Both employer and employee contribute monthly

Using Your Insurance

Present your medical insurance card or electronic certificate at designated medical institutions. The insurance portion pays from the pooling account; your personal portion from your personal account balance.

Commercial Health Insurance

Many foreigners purchase additional commercial insurance. Some employers provide it as a benefit.

Before Seeing a Doctor

• Confirm coverage: Check claim scope and requirements

• Bring documents: Valid ID and insurance card

• Check direct billing: See if the hospital has a direct billing agreement

Direct Billing vs. Reimbursement

  • Direct billing (直付): If your insurer has an agreement, settle without paying upfront

• Reimbursement (报销): Pay upfront and file a claim later

Making an Insurance Claim

For reimbursement, you'll typically need:

• Medical records (outpatient and/or inpatient)

• Medical expense invoices (发票)

• Diagnosis certificate

• Examination and test reports

Important Considerations

• Some public hospitals don't accept foreign insurance

• Verify which hospitals and conditions your policy covers

  • The insurer may review expenses and contact you about items outside the claim scope

Practical Tips

  • 💡 Critical: Always ask for a payment invoice (发票). Without it, you cannot claim reimbursement.
